Develops and implements an appropriate habilitative program for adult clients at Woods Services. Provides documentation, recording and reporting of client progress on all Individual Habilitation Program (IHP) goals and objectives and actively participates on the client's interdisciplinary team. Assists in providing a prescriptive, evaluative habilitative program for adult clients consistent with Woods Services Program Planning Guidelines as approved by the client's interdisciplinary team. Contributes to the development of the client's IHP. Assesses and utilizes instructional techniques, materials, curricular resources and professional consultation as necessary to develop, refine and/or enhance the quality of habilitative programming. Assists in developing and implementing individual behavioural programs as appropriate, in accordance with Woods Services Behaviour Management Policy. Maintains standard and consistent records of client behavior, work performance/production and progress on program goals. Prepares and submits to designated supervisor/program specialist any and all required reports. Works jointly with the instructional aide in the implementation of client programming and habilitative program site operations. Utilizes and maintains adaptive devices/equipment as stipulated in the client's IHP to assist the client in the acquisition of functional skills to enhance performance in specific tasks. Perform other duties as assigned.
Requirements
Must be 18 years of age.
Must possess a High School Diploma or equivalent.
A minimum of 1 year's experience in a habilitative/vocational setting serving adults with developmental disabilities is also required.
Must be able to read, write, enter dates and follow verbal and written directions, prepare written reports and complete written forms in English.
Must be able to verbally communicate in English and direct/respond to client's needs.
Must be able to verbally communicate emergency information in English.
Must be able to lift/transfer a minimum of 50 pounds.
