The CDC reports that 15% or one in every six children between the ages of 3 and 17 are diagnosed with one or more developmental disabilities. While many people with I/DD live independently and with few or no supports, a subset of them have multiple severe conditions that require highly specialized supports, increased access to primary care and careful care coordination.
Meeting the complex medical and behavioral health needs of this population in traditional settings is a challenge both for the patients and also for traditional healthcare providers. Using a population health management strategy, Woods has launched a primary care practice that integrates behavioral health and specialty services to improve access to and quality of care.

The Medical Center at Woods is a true Patient Centered Medical Home that serves as the centralized, coordinated, center of care for clients. Having a medical center right at Woods enables us to improve care coordination, provide extended service hours to reduce disruptive ER visits, and increase access to more advanced medical services. For many of our residents with complex medical needs, the ability to reduce environmental disruptions while providing them with first-rate medical care is truly a game changer.
