Medical Complexities

The CDC reports that 15% or one in every six children between the ages of 3 and 17 are diagnosed with one or more developmental disabilities. While many people with I/DD live independently and with few or no supports, a subset of them have multiple severe conditions that require highly specialized supports, increased access to primary care and careful care coordination.

 

Meeting the complex medical and behavioral health needs of this population in traditional settings is a challenge both for the patients and also for traditional healthcare providers.  Woods excels in serving individuals with medical complexities. Our round-the-clock, onsite medical care differentiates us from most other providers.

 

Population Health Approach

At Woods we see the most exceptional people with the most medically fragile and complex healthcare conditions, often coupled with behavioral challenges and genetic disorders.  This complexity requires a much different approach to providing quality healthcare.

 

Woods is organized around the principles of population health management meaning that we provide a comprehensive continuum of care – connecting prevention, wellness, education, behavioral health, and social services with coordinated and integrated healthcare delivery for the benefit of all the amazing individuals we serve.

 

The Medical Center at Woods

Using a population health management strategy, Woods has launched a primary care practice that integrates behavioral health and specialty services to improve access to and quality of care.

 

 

Services currently provided through the Medical Center include:

Neurology (pediatric and adult)

Psychiatry

Dentistry

Podiatry

Opthalmology

Optometry

Pharmaceutical services

Radiology

 

The Medical Center at Woods is a true Patient Centered Medical Home that serves as the centralized, coordinated, center of care for clients. Having a medical center right at Woods enables us to improve care coordination, provide extended service hours to reduce disruptive ER visits, and increase access to more advanced medical services. For many of our residents with complex medical needs, the ability to reduce environmental disruptions while providing them with first-rate medical care is truly a game changer.
